DM2 Risk Factors: Fat Distribution

Distribution of fat in the body affects the risk of development of diabetes mellitus type 2.  Specifically, fat stored in the abdominal region or fat referred to his visceral fat has been associated with higher degree of insulin resistance than fat stored peripherally on the body in the subcutaneous tissues. The higher the degree of insulin resistance the more likely development of diabetes is to occur.

To screen for abdominal obesity physicians utilize the waist circumference measurement. To measure waist circumference a flexible tape is placed at the top of the hip bones known as the iliac crest.  It is then wrapped around the torso and the circumference is calculated. A waist circumference of greater than 40 inches or 102 cm in men or for females a waist circumference greater than 35 inches or 88 cm for a woman is considered elevated and serves as a marker for increased cardio-metabolic risk.

Of note; the measurement of waist circumference becomes unnecessary in patients with a BMI greater than 35kg/m2 as almost all of these individuals with this type of BMI will have an abnormal waist circumference, regardless they are already at higher risk due to the amount of fat being carried on her body.

Due to the prevalence of diabetes affecting over 425 million people worldwide proper identification of all risk factors is of great importance as the majority of patients with prediabetes are unaware that they have prediabetes and are unaware that they are at risk of developing diabetes mellitus type 2.

Fat distribution analysis is something that any patient can perform at home as long as they are aware of the proper technique of measuring waist circumference.  Also, there are body mass index calculators online that can provide information to see if 35kg/m2 is exceeded. This is important as this may prompt the patient to present for medical evaluation sooner of which could lead to an expedited diagnosis.
