“Controlled Diabetes” – A Medical Misnomer

The phrase”Controlled Diabetic” does not exist and is misleading to patients diagnosed with diabetes. The definition of controlled diabetes is a Hemoglobin A1c (HbA1c)<7. The state of diabetes is both age accelerating and pro-inflammatory. The longer one is diabetic, the longer one is exposed to higher levels of circulating sugar (glucose) in the body of which exposes the organs of the body to accelerated organ damage. A reduced level of age acceleration or inflammation is by no means controlled or should ever be considered controlled. This is evidenced by the shortened life span of diabetics that is on average reduced by 10 years. A reduced life expectancy occurs due to multiple mechanisms. One of the mechanisms is the process of advanced glycation end products (AGEs). The AGEs are formed when excessive exposure of circulating glucose (sugar) becomes attached or glycated to proteins or lipids within the body. The HbA1c is a measure of glycation. All cells and components within the cells of the body are susceptible to the damaging effects of AGEs. Organ systems highlighting this process within diabetics are classically known as the eyes with diabetic retinopathy, the kidneys with diabetic nephropathy, and the extremities with diabetic neuropathy/diabetic foot ulcers/amputations. The formation of AGEs in themselves is damaging; however once created AGEs have the potential to advance disease progression by contributing to oxidative stress (inflammatory process) or activating other inflammatory pathways. An additional source of inflammatory promotion comes from insulin itself. Insulin serves as a source of inflammation by increasing pro-inflammatory cytokines from activated macrophages. Insulin regimens may be once daily or three times a day with meals with either a morning or evening dose of long-acting insulin also administered. Each point of insulin administration causes an increased spike in the occurrence of inflammation. The addition of insulin or high doses of insulin to maintain a HbA1c<7 in effect leads to a more uncontrolled state of diabetes by the mechanisms stated above.

The existence of diabetes itself translates into age acceleration and inflammation regardless of a HbA1c<7.
